To create an LPA document the donor must register with the Office of Public Guardian. This can only be done if the donor has the mental capacity.

Where the donor should live if they are too ill to live alone
The donor may also wish to appoint a Personal Welfare attorney. This individual must be over 18 years of age and will consent or refuse medical treatments or surgery on behalf of the donor.
